Congratulations for setting up your first appointment!

Please take the time to download and fill out the form appropriate below and bring it along to your appointment. This will save you time and speed up your first visit. If you do not have the time to fill out this paperwork at home, just come in 15 minutes earlier and we will have a packet ready for you at the clinic. What to expect

At home, prior to your visit

Fill out the initial paperwork: Acupuncture Form (pdf)
Bring your insurance card
Dress in loose comfortable clothes, or bring them along!
Possibly, avoid brushing your tongue - not your teeth! - for 24/48 hrs prior to your initial visit. The coat on your tongue is an important diagnostic tool for your DOM.

During your first visit

The first part of your visit will consist of an extensive interview with Dr. Carey. In particular:

Discuss the main condition you want to treat
Discuss your current health condition and health history
Tongue diagnosis, an important diagnostic element of Traditional Chinese Medicine
Pulse diagnosis, an important diagnostic element of Traditional Chinese Medicine
Palpation of acupoints, as applicable
Discuss your diagnosis
Discuss your treatment plan

The second part of your visit will be your acupuncture treatment with use of other therapeutic methods such as infrared, cupping, TuiNa, electrical acupuncture as they fit in your treatment plan.

We like to make sure that you and Dr. Carey have enough time to discuss your health history. Please plan 90 minutes at least for your fist visit. Your subsequent visits will be about between 45-60 minutes.
